Medical Statement

The purpose of this Medical Questionnaire is to determine if you should be examined by your doctor before participating in recreational diving with TSAC Inc. Please answer the following questions on your past or present medical history with a YES or NO. If you are not sure, answer YES. A positive response to a question does not necessarily disqualify you from diving. A positive response means that there is a pre-existing condition that may affect your safety while diving and we ask that you receive written medical clearance prior to participating in scuba diving with TSAC Inc. A list of medical practitioners with experience in conducting dive medicals is available from the website of the South Pacific Underwater Medicine Society (http://www.spums.org.au).

Irrespective of your answers to these questions, if you are a new member and are 45 years or older, TSAC requires that you present a current DIVE Medical indicating your fitness to dive. Existing members whose subscriptions lapse for more than 24 months will be considered new members for this purpose. A dive medical at least every 5 years is recommended for all club divers over 45 years of age.

Important Statement of Understandings

TSAC Inc., is a non-profit diving club managed by volunteers . All advice and actions are done in good faith. Any fees levied by the club are purely there to recover running costs. No members of the club are financially rewarded for their services. As a result, the committee and agents of the club (including but not limited to the committee, dive organisers, and boat skippers) can take no legal responsibility for their advice, actions or omissions, negligent or otherwise.

I release TSAC Inc., from all claims which I have, now or later against TSAC Inc., whether for: property damage, personal injury (including death) or otherwise, and whether occasioned by negligence, breach of contract, breach of statutory duty or otherwise, suffered as a result of or in connection with any of my activities with TSAC Inc.

I indemnify and will at all times in the future indemnify TSAC Inc., against all proceedings, actions, claims or demands made by any person for : property damage, personal injury (including death) or otherwise, and whether occasioned by negligence, breach of contract, breach of statutory duty or otherwise, suffered as a result of or in connection with any of my activities with TSAC Inc.

I hereby apply for membership of the Tasmanian Sub-Aqua Club Inc., and as a condition of membership agree to be bound by the release and indemnity above, and the terms below.

Your presence at any club diving activity is not in any way an endorsement of your diving skills and/or qualifications. You hereby acknowledge that diving is a dangerous sport and realise that you must act in a responsible manner at all times. You are wholly responsible for your own safety when diving with the club and are expected to use your own training and experience to determine whether you can safely participate in any club activity.

You agree to comply with all reasonable instructions provided by the dive coordinators to facilitate the smooth running of club dives/events.

It is your responsibility to ensure that any equipment you use, whether you own it or not, is in working order and that it will allow you to safely participate in any club activity

TSAC Inc. encourages all members to carry personal insurance to cover any eventuality arising from diving activities.
